Dental implant supported reconstruction of the edentulous maxillary arch presents a unique challenge due to its specific anatomy. The size and health of the maxillary sinuses can often dictate the surgical options for placement of conventional endosseous implants. The implant supported fixed hybrid, or “All-On” solution is a predictable and well substantiated treatment protocol for patients with sufficient basal bone to accommodate implant placement between the sinuses, whereas patients with inadequate residual bone must often rely on more complex procedures such as zygomatic implant placement.  In this seminar we will review the various treatment options for the fully edentulous maxilla, and will present a novel approach involving the utilization of prosthetically driven CADCAM custom 3D printed subperiosteal implants for patients with very severe maxillary bone deficiency.
